Is my loved one eligible for GUIDE?

 

✅ Diagnosis of Dementia: Any type qualifies (Alzheimer’s, Lewy Body, Vascular, etc.).

✅ The "Red, White, & Blue" Card: They must have Original Medicare (Parts A & B).

✅ NO Medicare Advantage:
If their primary insurance is a private plan (like Humana, Aetna, or UnitedHealthcare), they are not eligible at this time.


✅ Lives in the Community:
They must live at home, in Independent Living, or Assisted Living. (Those in a Skilled Nursing Facility/Nursing Home do not qualify).


✅ Not in Hospice: They cannot be enrolled in Hospice. However, Palliative Care is perfectly okay.
