# The GUIDE program is designed for families like yours. See if you qualify!

## **✅ Diagnosis of dementia:** Any type qualifies (Alzheimer’s, Lewy Body, Vascular, etc.). **✅ Original Medicare (Parts A & B) is their primary insurance** and they do not have a Medicare Advantage plan. If their primary insurance is a private plan (like Humana, Aetna, or UnitedHealthcare), they are not eligible at this time. **✅ Lives at their home, with a loved one or in assisted living.** Those in a Skilled Nursing Facility/Nursing Home or currently in hospice do not qualify.
