This Privacy Policy explains how Ianacare, Inc. ("ianacare," "we," "us") collects and uses information on our GUIDE eligibility pages at info.ianacare.com. It covers the forms you use to check eligibility for the Guiding an Improved Dementia Experience (GUIDE) Model, a Medicare program. Our mobile app and main website are covered by a separate privacy policy.
Information we collect
- Your contact information: your first and last name, email address, and phone number.
- Information about the person you care for, to check GUIDE eligibility: the patient's first and last name, date of birth, and Medicare Number (also called the MBI).
- Automatically collected information: device and usage data collected through cookies and similar technologies to operate the site and measure our advertising.
How we use your information
- To check whether the person you care for may qualify for the GUIDE program.
- To verify Medicare eligibility directly with Medicare.
- To connect you with a CMS-approved clinical partner in your area that delivers GUIDE, such as JVS Health, Care Dimensions, Author Health, or Translate Neuroscience.
- To contact you by email, phone, and text message about your eligibility and next steps.
- To operate, secure, and improve our site.
How we share your information
We do not sell your personal information. We share it only as described here:
- With your clinical partner. We share your information with the CMS-approved clinical partner that delivers GUIDE in your area so they can confirm eligibility and provide care.
- With Medicare. We use the Medicare Number to confirm GUIDE eligibility with Medicare.
- With service providers. We use trusted providers that help us operate, including eligibility verification, secure hosting, customer relationship management, and text messaging. They may use your information only to provide services to us, and providers that handle health information do so under confidentiality and, where required, HIPAA business associate agreements.
- For legal and safety reasons where we believe it is necessary to comply with law or protect people's rights and safety.
Text messaging. No mobile opt-in information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are not shared with any third parties.
Health information
The patient's name, date of birth, and Medicare Number are sensitive. This information is encrypted in transit, access is restricted to the people and providers who need it, and it is handled under applicable HIPAA safeguards. We never use this information for advertising and we never send it to advertising platforms.
Advertising
We use advertising platforms such as Meta and Google to measure how our ads perform. We do not share your Medicare Number or the patient's health information with these platforms.
Your choices
- Email: unsubscribe using the link at the bottom of any email.
- Text messages: reply STOP to any message to opt out, or HELP for help.
- Access, correction, or deletion: contact us using the details below.
Security
We use organizational, technical, and physical safeguards designed to protect your information. No method of transmission is completely secure, so please avoid sending sensitive information by email.
Children
Our GUIDE eligibility pages are intended for adults and are not directed to children.
